US Robotics Sportster Model 0484 Driver. This is an OEM modem, made by 3Com but supported by the manufacturers that distribute it. This isn't only a driver file - it's a flash upgrade for the modem, from X-2 to V.90. I got this file, PYTV90UP.EXE from Dell tech support, and it worked easily to flash the modem. The only problem was that the instructions said I'd need to use the included driver on the floppy disk that the file creates - but in fact I didn't need this driver, because there was a built-in driver in Windows 98 Second Edition that got installed automatically. The Win 98 driver worked fine after the V.90 upgrade.
