US Robotics 0766 or 3CP5699A Driver. PCI modem with model 0766 marked on back and has white label with a product number of 3CP5699A. These drivers are right off the installation CD's folder for Windows NT. There are 4 folders in the ZIP file and the one marked "265699a" SHOULD be the one to use, but I've not tried.
