Jaton Wincomm56 008319 Driver. This is a Jaton modem with Cirrus Login chipset CL-MD5620DT-QC-A. It shows as a Cirrus X2 Fax Voice CLM95 in device manager once loaded. I tried several other drivers listed on this site and none worked. Lucky for me I managed to find the original CD. I wish you luck.
