These drivers were intended as an upgrade from version 1.2 to 1.4. I modified the original file, V14A.exe, to make it easier to use as a full installation also.
For a full installation the modem MUST show in device manager as a 'PCI serial controller'. On first boot up after installing it you'll get the 'new hardware found' box. Keep hitting 'next' and it'll eventually finish and say it couldn't install any drivers. That will force it to show in device manager correctly as the PCI serial controller. If you've attempted previous installations that haven't worked you may need to go to add/remove programs and remove them.
MDM28PSVD.exe is a self-extracting zip file. When running it, it will default to extracting the files to C:\. Go ahead with that. It will create a folder named '336DSVD' and dump all the files there.
If you're upgrading from 1.2 to 1.4 go ahead and run 'upgrade.exe' from that folder. For a full installation run 'setup.exe' from the folder. Go ahead and choose the installation defaults.
When you reboot after the installation a message will show about a video conferencing application being installed that wasn't compatible with 98. It'll say it fixed it and you need to reboot. Go ahead and do that.
These drivers were originally intended for 95 and the installation process expects to find some files on the 95 cd that aren't on the 98 cd. These files are:

vvexe32.exe
wsvv.vxd
I've included those files in the 336DSVD folder. So when it asks you to insert the 98 cd, it won't be able to find those file. When it asks where else to look, browse to the 336DSVD folder and it will find them there.
It will also need to copy other files that ARE on the 98 cd.
After the successful installation you need to run the patch. Open the 'patch' folder in 336DSVD and run the 'setup' command. The patch is supposed to reduce the liklihood of disconnects while on the internet. The original patch file was named TELPT05.EXE. For convenience I extracted it and included it with the driver download.
After the installation two modems will show in 'modems' in 'control panel'--MODEM GAME AND DATA LINE and MODEM VOICE AND FAX LINE. That's normal for this modem.
