Motorola SM56 Data/Fax Driver. There are two files zipped into one. Unzip them ant they will fit on two floppy disks. From there it's easy.
File Contents: I tried these and the data/fax/voice drivers on a SM56 chip with 62412-51 on it and only these drivers worked with that chip.
