Rockwell V1456VQH-R5 Driver. I searched for DAYS AND NIGHTS, what costed me so much time was, it wasn't built by Rockwell, it has a Rockwell chipset, but Rockwell was the only thing close to a name I could find on the modem! If you're numbers match the numbers I have listed and you do it right, it will work! Extract it to a floppy, remove your modem, when adding it back, tell Windows you want to select it yourself, and boom, you are in business. Just remember to change which modem to use to connect to your isp in the connection properties, that threw me off for a while, if you don't do that it won't show youv'e got a modem, but you already know that
