Zoltrix FMHSP56PCI or FM-1789 Driver. The board has FM-1789 written on it. mine is Rev 1.5 (others should work as well).
File Contents: The real device name is FMHSP56PCI, but there is no link to such a name in Zoltrix's driver download page.
This is the correct driver download page for all OSes.
