Intel ENF656-GPCI-INPR or Intel 536EP Driver. 56k v.92 Fax/Modem Intel chipset with ENF656-GPCI-INPR label on the modem but no drivers I downloaded would work. Fortunately we found the original drivers and they work great, so here they are.
